I am new to this website and your posts are really informative and impressive.

I sent you a friend request and if you could accept that please.

I am a candidate with TWO root canals done (teeth 30 and 31) about 3 years ago and I recently learnt that at the bottom of teeth 30 there is an infection. I have some picture that I would like to share. I dont know if I can share them here or if I should share them in private via email.

A week ago the pain was unbearable and then I started taking Amoxycillin 500 and Ibuprofen (only when needed). I also did a lot of home remedies - salt water gargles, turmeric powder application etc.

The pain is gone down almost 75%. But there is still some irritation under the tooth.

My options are either to retreat the root canal or get the extraction done.

If I get is extracted then that will leave space between 29 and 31. I dont think I can afford an implant really.

I was wondering if you could take at look at the x-rays I have and suggest. I am assuming that you would ask me to decide for an extraction but I still want to make sure if that is what you would suggest after looking at those x rays.

Hi, and welcome to Neuro Talk. I know Bryanna will post to you. She does indeed know alot about root canaled teeth. This is my take on it. re-treating an infected tooth is a waste of money. In fact root canals in general arn't so good as part of the infection is left deep inside the small crevices, and this is what happened to you. I have a besnit. A two tooth partial that fits just wonderfully, and was not that expensive. I didn't have the money for an implant either. I wish you all the best. ginnie

Right now my pain is almost 20% of what it was last week so before I make a decision on anything I want to evaluate all options.

Yes, post the xrays here on this forum or pm them to me.

However, since all root canaled teeth harbor infectious bacteria.... and because there is no way to "cure" the infection... and because the infection is likely to spread.... I believe the only healthy option pertaining to infected teeth is to have them removed.

Any topical solution or medicament will not cure the infection. Any dental procedure both surgical and non surgical will not cure the infection. Any antibiotic both oral and IV will not kill the bacteria inside the microscopic canals within the tooth.

If you still want me to look at the xrays..... that's fine. I'm happy to do that for you.

After my post I was taking Augmentin Tab, Moxigil 400 and Arflur 3D (Indian origin medicines) and right now I dont feel any issues with that tooth. That said, I know it was an infection and I cannot let it sit like this.

I have to take care of this ASAP.

The device is small, and holds two teeth that look very natural. The plastic curls around the teeth in front and in back. Besnit. If I got the spelling wrong this time Bryannna will just have to put me out of my misery! Love the device and I can indeed eat with it but I am careful too. No taffy, steak, that kind of thing. My spaces did show when smiling, and this brought back my self confidence. I did not re-treat the bad teeth, nor go for expensive gum restoration. This seems the better way to go for me. I still have one RT tooth left. I will get that out as soon as I can. ginnie

Thanks for posting the xrays.

As suspected, both of these teeth look to have radiographic pathology indicating infection. If this is your lower right side, assuming the wisdom tooth has already been removed, these two teeth are #30 and 31.

The last tooth #31 looks to have a cyst extending out of the distal (back) root. The periodontal ligament surrounding this tooth appears to be widened which would also indicate that bacteria has traveled beyond the tooth.

The first molar #30 also has a cyst (shows here as a black circle) extending out of the distal (back) root.

Your replacement options are limited if both teeth are removed. A Nesbit appliance as described by ginnie would not work here because you would need to have an anchor tooth on each side of the appliance to hold it in place. You may be a candidate for a unilateral partial denture or a full partial denture if you have teeth missing (and not yet replaced) on the other side of the lower jaw.

Not everyone is a candidate for partial dentures, it depends on the individual anatomy. I suggest that you speak to your dentist about these replacement options. He may suggest dental implants which could be a viable option but you need to keep in mind that implants are a bit risky when they are replacing root canaled teeth due to the bacteria in the jawbone.

Thanks so much Bryanna. I'm finally booked in to get the tooth extracted by a reasonably priced dentist who will also do the implant.
I'm also having deep cleaning done on all my molars to repair any gum desease. Do you generally recomend deep cleaning - It was the first I'd heard of it? thanks again. Mark

I Just wanted to add, that deep root cleaning does not hurt these days. Mine had no shots at all. New proceedures have been developed. They use a pokey like took that gently places the numbing meds inbetween and all around the tooth. No stress, No pain, all goes well. Ask Your dentist how they do this. Hopefully they can use this new way for you. ginnie

Your welcome Glad to hear you are scheduled to have that tooth out.

Deep cleanings are therapeutic and reduce some pocketing which ultimately reduces the inflammation. The key to this treatment being successful is actually the oral care that you do at home. Your tooth brushing and flossing must be thorough and consistently thorough once the cleanings are done. Otherwise, the improvement from the treatment will not be long term and you will be right back to where you are now.
