Default WC questionaire

Today I received a letter advising me that I have a WC exam appt. in Feb. included with the letter is a questionaire with no instructions. I don't know know whether I am expected to fill it out now or do it just before the exam and bring it with me. Yes, I will call them, I tried 4 times earlier today, but you would think they would include instructions and clarifications.

The questionaire requests the name of my doctor. Which doctor? My primary care, my PM doctor, or one of the many other doctors I've seen?

It also asks for my medications. Do I list OTC as well?

WC refused to pay for me to see a CRPS specialist in another state so I was unable to get a prescription for Low Dose Naltrexone. Because I was afraid I would never walk again I ordered LDN from abroad. I am currently trying to find a local doctor to prescribe it for me, so hopefully this will not be an issue listing it.

The questionaire asks about surgeries. I am anticipating having brain surgery for
an unruptured brain aneurysm found incidentally after this CRPS nightmare started. I hoped to have the surgery before the scheduled exam, but now I am wondering if I should. (I don't know if I was blessed by the accident that led to discovering the aneurysm or cursed)

The chronic pain from CRPS has altered me to the extent that I can not plan well. I used to be a person with wherewithal. Now I have none.
