Default SSA 455 help!

Sorry if I double posted. I'm a newbie.
1. I received the ssa 455 form. Is a handwritten form machine read or should I fill out a form online or scan mine in?

2. I had gotten advice to attach ALL my appts (it's a lot) but from what I'm reading (which isn't easy due to my vision), I should limit to space provided & not fill out the last item with extra detail or attach anything. Correct?

3. My dx has severely deteriorated into a related dx that qualifies for the compassionate care allowance. From what I understand, I should not change the dx (I still meet criteria for my original dx) or add new dxs. Correct?

4. If I'm Correct on #3 above, my original dx goes by several names & I cannot find anywhere the papers that were used for my original application for reasons I don't want to distract you with.

5. Pls reply ASAP, as the form was back dated by the time I got it and they said I can turn it in this week. Thank you.
