yes but I saw it too late, I panicked when I got the form and called my nuerologist but he ws not in so I call my psych and was in such a major paic she said she would help me out and she mailed it this past month, there is no way to get it back is there ? I want to cry and I can't sleep and I see an ENT in the morning for a terrible fall I took, haven't been using my walker......I did send an email to the disability lawyers I used last time to get ssdi in the first place, I was supposed to see a judge, but he wrote our a letter as to why I should have full approval, will that come in handy to use?
