In addition to what's been offered, all I can do is repeat my suggestion to see a certified myofascial trigger point therapist (CMTPT). You don't need a referral, but insurance may not cover it.
OR... see a myofascial pain doctor. I don't know if you would need a referral or not; your insurance carrier would know, and should also have a list of specialists in your area. That would get around your deaf doctor problem as well.
