Hi e1ectric,

I had no idea how many mistakes are contained in medical records until I requested mine for filing for disability. The cause of death of my father is listed incorrectly. The cause of death of my mother is listed incorrectly. I KNOW I did not give them the wrong information. They charted that I had GERD and I have NEVER had gerd or any symptoms of gastric problems of any kind at any time. They say that I was taking medications that I was never taking, referred to doctors I was never referred to, etc. etc. And the list goes on. Symptoms that I DID complain of were not charted at all. They write what THEY think is relevant or their interpretation. I do have medical records from one of my docs that is TOTALLY accurate and complete. That was very refreshing to see.

Hope you get your records corrected.
