My SSDI attorney specifically advised me not to add additional pages with detail - her point being if I was able to sit and devote many hours to doing a very thorough and detailed report, it might appear I was able to focus and could hold a clerical position with no problem. I hope it's not bad advice. She has stellar ratings on Avvo.

Anyway I see on page 8 there is a spot to fill in your name, address, etc. I see no signature line though. Am I missing something? I thought with something of this gravity, there really ought to be a place to sign & affirm your statement.

I know I can call the social security number on monday or my lawyers office - just thought I would ask my friends here first.

It took me weeks to complete mine with all the added pages. If they think I can hold a clerical position just because I could "add" detailed pages, they do not know how LONG it took me to do that.

They know how long it took me to complete the application because I did it online and it took weeks. I guess they can track how often I signed back in to work on it.
