Dr. Kalish is recommended with the following caveat. He is mostly involved with his teaching so sometimes I have to bug him a bit. He is probably second only to Dr. Hinz in knowledge of this method and they talk a lot so he will dig up the answers to questions he doesn't know.

It's hard to remember how long it took to respond but it was about five months I think. Of course, mine was slower because I submitted the tests every couple of weeks.
