I think the ER will not be much help. You need to find a successful treatment for your neck. With it such a mess, everything else will be hard to diagnose and treat. Have you been seen by a Physiatrist (Physical Medicine and Rehabilitation) ? The trembles could be a symptoms all their own or they could be from the anxiety for being in pain. Nobody can say.
