Hello Kimberly,Abbie is correct,but that form like all the other forms for physical trouble i got through my attorney, then doctors would fill them out my disability attorney would go through them before sending out to SSA.This is the first time i heard a disability attorney not knowing of this form,unless his specialty is not on disability.If he isn't get one that is,for your own good!GOOD LUCK this attorney sounds like an ambulance chaser!
