Default CDR Question

I am on SSDI for bipolar disorder and ADHD and I just got the dreaded CDR long form after just over 3 years. I am curious how to fill out the typical daily activity portion, because on a good day, I have a normal day, but others can go from one extreme to the next. What is the best way to respond? Can I just say that it varies greatly? I am for the most part stable and I see my dr every couple months or more and always take my meds as prescribed. I am panicked, because if I lose medicare I'll have to start over, meaning psychiatric hospitals and generally being out of control. I don't want to do that again.
