I think what JoMar said is correct. Any time there is involvement with a nerve root as your indicates, that is painful! Doesn't seem like your neruo surgeon was on the same page. Also if you didn't like the attitude, don't hesitate to seek another opinion yet again. When two doctors don't agree maybe another can be more proficient at explaining all that your MRI says.

It is also my understanding if the injections don't work the first couple of times, they won't work trying them over again. Sometimes the doctors do that to continue on with conservative treatment.
Another option would be to seek a doctor that knows about Ketamine infusions. I had this done several times. I had hoped to avoid surgery. This infusion did help, for months at a time. I wound up having to have surgery anyway. Different condition however than yours.
I also think the MRI report was a little different from how the normal ones are written. Words like Mild, Moderate, severe, relating to all the vertebrae is absent. That is most like what they do, which tells the patient, exactly how bad that particular vertebrae is. Don't hesitate to ask for a better explanation from both doctors, and relate to them that there is a difference of opinion. You have to feel right about what you are being told, and know exactly what your MRI means before any patient could make a choice about surgery etc. I wish you the very best outcome in what ever decision you make.
