I've seen a few different doctors and they all have some information on me, but I don't know that any of them would take the time or feel they have my full story to fill out that form.

Some might require a small fee. Some might want you to undergo RFC testing (usually by a physical therapist) and then use that info to fill out the form--if you have to go out of out of pocket for the testing it might run a few hundred dollars. So long as you don't ask them to get it done within an unreasonably short timeframe, most will accommodate you. You are entitled to subpoena them (which isn't done very often and is considered obnoxious), so filling out a form that'll take them 10 minutes isn't a big deal.

One more question on the psychological exam. Is it more talking or more of a pen and paper test to guage my anxiety/depression? I'm just not sure what to expect.
