Default kneecap sensitivity post knee injury

Hello not sure if this forum is best or the rsd one.

I had an bilateral overuse injury in the patella femoral joint 6 months ago aggravated by agressive early physio.

I stopped the exercises and the mechanical pain (walking/going down stairs) went away.

What i am left with is sensitivity in the kneecaps which is variable. The sensitvity is to cloth and cloth friction, such that wearing trousers is uncomfortable, and i can feel the bedsheets on my knees, i will occasionally get resting pain and buzzing/burning on the knee caps.

Whats interesting is that occasionally i will get a heat rash on a kneecap with heat for 30-60mins this might happen twice a day per knee cap or it might not happen for weeks.

This sensitivity has completely dissappeared for 1 week to return and has gone into substantial remission twice before returning. I did use 5 mins of ice per knee + biofreeze each night for month for pain relief and am concered that this damaged the nerves?

A specialist physio says that it is a neural dysfunction that is common in pfps and should go away, she said athough it might seem like a watered down version of rsd it is not.

She reconends letting it settle naturally or alternatively using capsin cream plus cardio to help it on its way. I have held back on the capasin cream because im worried its similar to the biofreeze but am getting desperate.

She is worried that an issue in my lower back might be influencing it ( i have recently developed mild bilateral sciatica).

Any thoughts on this are much appreciated.
