I am not sure the doctors really have a handle on what is and is not a centralized pain case. It seems as if they will apply the label when the severity and locality of the pain moves beyond what they expect or are comfortable with. I have been trying to figure this out myself.

From what I have read in Practical Pain Management peripheral neuropathy can most definitely migrate to centralized pain.
