Neuropathy and Myopathy are two different conditions--the former is damage to nerves, the latter, damage to muscle--but of course, one can have both simultaneously, and the most common causes of seeing both together would be autoimmune phenomena or toxic exposures.

Given your other conditions, has the possibility of celiac/gluten sensitivity been broached?
