You should get a doctor or nurse to help you dress that wound.
Clean it every day with hydrogen peroxide...which bubbles up and shows you by how much it bubbles, how dirty it is.

There are toe bandages, and a podiatrist could show you how to take care of it, so it won't get infected.

Type II patients don't get severe infections as often as type I patients. The length of time you have had type II is also a factor.

I use hydrogen peroxide on cuts etc. all the time.

Consider with all that metformin you use that you may need B12 supplements as well. Low B12 is common in India (due to diet), so you can get testing and proper oral dosing if you are becoming low. Metformin blocks B12 absorption, so do get that tested and squared away. You should be at 400pg/ml at least.
