I received my social security disability CDR (short-form) yesterday. I think I answered most of the questions correctly except for question #5b I entered:

1. Counseling 05/15
2. Counseling 05/15
3. Prescriptions 04/15

Any feedback on how I answered question #5b will be greatly appreciated.
