I am so sorry to hear about everything you have been through and the pain you currently suffer. What you describe sounds very much like Thoracic Outlet syndrome.

First off you need to find a TOS specialist and likely a vascular surgeon since the symptoms you describe certainly sound vascular in nature. Where do you live? There is also information on the TOS forum about workmans comp that you might find useful.
