I think you need to find an ENT, maybe at a teaching hospital to do the diagnostics.

Keep in mind that there is no cure for common tinnitus. Unless you have some extremely rare condition, you may just need to learn to live with your tinnitus. My ears are ringing like a bell right now. I just ate and they will be this way for a hour or so before then settle down to a less intrusive level.

What makes you think your symptoms are related to a perilymph fistula ?

Have you had CSF leakage ? That is usually associated with head injury fistulas.
