If your back and hip are the impairments that are keeping you from working, then you shouldn't have listed the other impairments on this latest application IMO.
If you live in a state where you're not eligible for Medicaid, then perhaps see if there is a teaching hospital that will help you--many have charitable programs. You could start by calling the Red Cross.
Another option is Vocational Rehabilitation Services. They will send you to their own docs to determine if you have functional limitations to see if there is work you're capable of.
