This includes the disease itself as well as treatments and medications affecting your ability to work. Depending on how your peripheral neuropathy affects you, make sure you mention how it affects your ability to sit, stand, kneel, bend, reach, climb, lift, walk, due to numbness, pain, etc. also how medications affect your ability to concentrate, make you drowsy, how pain affects you, etc

Hi my doctor does have it down on all he's notes that I have Gait abnormality and Paresthesia and lower extremity weakness and gait instability. Thank you for your reply
