This is a difficult situation…

I think you should try his office again… be polite and explain the deadline, even if you already have. You could also politely ask about the delay. I know it's hard to be polite when you're so upset though

You may want to try to get an earlier appointment with him in case the paperwork with the Silenor (or a generic form) falls through so:

1. You can get a script for the Doxepin, get coupons from GoodRx online, and have it filled without using your insurance (it's very inexpensive that way). It may be a much easier option than going through insurance. Just explain it's the generic, and he will use his desk guide to look up the dosing equivalents before writing the script, OR

2. You can discuss getting a prescription for a drug with a similar action that's on your formulary.
