I hope you can clear things up when you see him in person.
You could ask him to write you a script for the generic for Silenor, and have that filled using a coupon from GoodRx.com (the prices are cheap, but depend on the particular pharmacy you use). You can't use insurance when you use those coupons, so you wouldn't need to deal with those faxes or paperwork.
