I think the best time to document acute symptoms is usually right after they subside when you're no longer in acute distress, but can remember them clearly. Sometimes doing that is helpful in distinguishing exactly what's going because we otherwise want to forget those awful experiences.
Anxiety increases with depression, so you maybe getting hit two-fold even though you're mainly experiencing the symptoms of the anxiety right now. But obviously that's where you need the relief the most
I know that your pdoc has offered lots of meds. I hope that with your input, she can narrow it down to one or two plans so that she's not muddying the waters and you can decide on an option you like and get started on feeling better
