I have a similar issue and I believe it to be occipital neuralgia as well, a result of muscles that have spasmed for so long that it has caused nerve irritation.
I also have a condition where my head tilts slightly (unnoticeable to anyone other than a professional) to compensate for a visual dysfunction. It's kind of similar to vertical heterophoria if you wanted to look that up to see if that is fitting for you as well.
Sorry that I don't have any answers myself, but I'm hoping fixing my vision will solve the migraines that are causing the neck spasms and thus leading to chronic neck pain, nerve pain, etc.
