I forgot to write that since the VNA will be seeing your MIL one of the nurses should be able to pick up on her SOB when they see her. They should be able to report that to her doctor...

The RN/RNs can also kinda push the pill case on her and help her fill it, so hopefully she will get into the habit of using it, but there's no guarantee of that. Some people are just stubborn.
