If you continue to have trouble with the nurse practioneer I'd ask
at least one of the patients that uses them to talk to the doctor
about how you wont be able to come if you don't get the
order. And that wont be good for them. That she/he will have
to do the toes for them.

I am guessing this will get them off their butts to do the work.
