Low MVC usually indicates a B12 or folate issue. Key nutrients for blood production are B12, Folate, Iron, Coper, Vitamin D, zinc and B6. Bowel/gut issues need to be explored. H.pylori bacteria which causes ulcers can impact both bone marrow and malabsorption of nutrients.

The B vitamins are key nutrients for Blood/ bone marrow and the central nervous system/sleep/anxiety. Keep in mind that medications can impact both and also deplete some nutrients.

You may want to check your past nutritional deficiencies and ensure they've been corrected.

I too think it's unlikely that it's CLL but you may want to consult a hematologist.
