Sometimes it takes a while for your system to go back to normal after you have been using large amounts of laxatives for a long time. Your symptoms may get better just from stopping the OTC laxatives
Lactulose doesn't always produce a BM every day. When was the last time you went? Anywhere from 3 times a day to 3 times a week is considered "normal."

I hope the kiwis help with your IBS symptoms. They sound promising

Gabapentin is an anti-seizure medication that is used for neurological pain, but it can be used off-label as a mood stabilizer, but usually as an adjunct, not alone. It doesn't work for everyone.
I used to take it in VERY high doses to treat my neuro pain from the MS, but it stopped working for that, so I stopped taking it. I take it in a MUCH lower dose as a mood stabilizer now, and it works well for me. It's the only med we can move up and down to treat milder episodes of hypo/mania because I'm maxed out on everything else.

Why do you say you are unstable? I thought that the Aplenzin was working well? (((HUGS)))
