Hi Better ~ I have a myriad of neurological symptoms that have plagued me since 2004. This came on suddenly for me and sound pretty similiar to yours, but I did not receive a concussion. I have taken some pretty hard knocks from falling off of horses as I have ridden all my life, but back when, there was no recognition about concussive after affects. If it hadn't been for my hardhat I would not be here today.

Anyway, in 2004 while buying a horse, I felt suddenly dizzy and unable to orient myself in the stall. I was okay driving home, but the dizziness persisted. Eventually, after exhaustive neurological testing and finding nothing wrong there, I moved on to see a neuro-otologist. I seemed to fit Menieres Syndrome and he did the specialized tests which confirmed this. During a Menieres attack, my brain is totally focused in keeping me upright and finding where I belong in space that I experience disregulation and depersonalization.

The fact your neurologist and other doctors are not picking up on what's going on is due to the fleeting nature of these episodes. They really need to see you when this is going on, which is usually impossible.

I like to think that what you experience when you go in and out of your symptoms as called "auras". Auras can be part of migraine or seizure disorder. And this could be temporary and may resolve itself and just be from being kicked in the ear/head region.

Have you been to a seizure specialist (not a regular neurologist)? The gold standard for most seizures is the EEG test, when the doc puts electrodes on your scalp and you lay down (totally painless) and the electrodes track your brain waves. Not all seizure types are discovered this way though.

With the kick to the ear, seeing a neuro-otologist and having those specialized tests, such as an ECOG and a hearing test. Also there is the VNS test for balance as well. Those three are pretty comprehensive for testing inner ear issues. Then when I had my MRI, they tested for an inner ear tumor and when the MRI came to the ear portion they injected me with GAD to highlight that area to rule out a tumor (not to scare you as not all tumors are bad). The kick to your ear may have jostled part of your inner ear where balance is located.

Migraine associated vertigo/seizures/menieres syndrome are all inter-related, per the specialists I have seen, they all seem to overlap somehow, but no one really understands the working part of all that.

Your feeling a sense of Depersonalization/derealization/and sense of time being off episodically could be an aura of migraine or seizure disorder.

Another thought, along the line of psychiatric, that kick in your ear/head could have triggered a traumatic memory............ I'm reaching here, so sticking with therapy for now to help manage these symptoms is key to helping you cope.

You are on the right track, ruling out everything and having baselines medically conducted. I think there are a few more highly specialized tests you may need. Consider looking up a "neuro-otologist". You can find a list of them on the VEDA which is "vestibular.org". I recommend a neuro-otologist inlieu of a regular ENT as they are highly trained in the neurological manifestations of the inner ear. From there, if you have those specialized tests done, they can also direct you to other specialists to help you go further with this. Thinking you may not have seen the "right" specialist. Keep looking and never give up.
