You mention several falls in recent months. I wonder if you've given yourself enough time just to get over those falls? As we get older, it seems to take more and more time to get over a fall.
Also, I have Type 2 diabetes too and understand that diabetes complications can come along even when it's being well controlled. Unfortunately.
There may be an answer to your continuing fatigue. I hope your doctor is checking your diet and making sure you're not deficient in some important nutrient.
