Although I can't give any helpful information, I can say it's too bad you're being bounced around so much in your efforts to find some answers to your health issues.
My own experience with back pain, which goes back decades, is that it comes and goes, and that sometimes really painful episodes lasting weeks will very very gradually get better if I just try to be sensible and stay away from any activity (or position) that is very painful--and so the "answer" is time and more time.
It looks as if several of the listed conditions might be helped by therapy/counseling. I wonder if you've gone that route? It might be worth a try.
