I sounds like more rounds of therapy might be needed.

What are the therapies they are doing for you , or having you do?
Does it seem to be helping, is the bigger question?
Is the therapy working on your upper body at all ?- neck, shoulders, armpit/pec minor, posture?

If they only focus on where the symptoms are in your arm and hand they won't get to the core of the problem.

But-
If you have NCV/EMG testing that clearly shows proof of CTS and Cubital tunnel entrapments you might need surgery for one or both of those to see some improvement.

Those can be adding to your TOS pain levels and the hand dysfunction - it's hard to separate it all out when there is multiple "crush" involved.
