Smile Prepare Form SSA-3368 Adult Disability History for the interview

I have found that preparing Form SSA-3368 Adult Disability History for the initial inteview more beneficial that an actual application. You have to call the SSA 1800-772-1213 and ask them to mail it. Tell them your computer has crashed otherwise they try to get you to download it. It can not be downloaded and leads into an application. Gather all the information on the 3368, MRI's, CT;s, Xray's etc. This form list everything you need to take with you. The interviewer will take all the info from it to file the application and then he will fax the 3368 to the state office. This gives the state much more information than the application and avoids delays. If at all possible, DO NOT FILE ON LINE, make an appoint for a personal interview. The computer can turn you down with no common sense or heart. Also if you happen to know a STATE rep or senator, make an appoiint with him and ask him if he has a contact in the state office. If so ask if he could call him to expedite and give your application special handling.TIMM
