On my A4 Daily Diary I then record my program together with an observation factor scale 1 to 10 at the right side for Eyesight =E, Cognitive =C, Mobility =M, Shaky = S, Dizzy = D and Vitality = V.
I also show a sleep factor for the previous night = PNS

This can program of mine can only be a guide for you as no two with PD have exactly the same symptoms.
Some ideas you may consider. I set my calendar alarm on my Mobile for the 5 times of the day I take medications, store the 5 lots in small containers and when away from home take the appropriate ones and always carry a bottle of water.That's all I can think of for the moment.
