methylfolate is no longer available OTC--- Merck decided to make it only for RX (not the FDA) to make more money: But folinic acid is close:

All 3 of these vits are activated forms. So make sure you buy these only to be exactly like Metanx. You can figure out the doses. The P5P is okay in 50mg amounts. Don't cut them, they are enteric coated. If that 50mg bothers you somehow, take it every OTHER day.

For PN there is another thing to try that works well for some people:
