Going to be a bit of a long read but well worth it IMO. The very first part points to the nature of neurological diagnosis. It's not like looking at a skin rash or a bump and being able to physically see what's going on. It would seem much hinges on the experience of the Neurologist. If one can get at least a tentative diagnosis and then find a Neuro that specializes in what your diagnosis says you might have it makes a big difference to treatment and outcome.

And it also points to the fact that many neurological problems have symptoms but they've not yet found a definitive cause for the problems. RLS is one of those. They seem to have found several triggers for the condition starting up. And then there's PN with no overt cause or underlying physical disease process. That's what I have, supposedly.

The Neuro that I saw only told me that he sees many people get PN as they get older and that it's just a part of aging for some. Obviously, after being on this board and reading and following links and doing my own research I'm finding his answer isn't good enough and really doesn't ring true.
