Default Medical Records

I'm facing more surgery.....and based on my past experiences I'd like to go in "armed". I would like to get copies of the last two attempts to administer general anesthesia to me......and the failure of same.....I'd like to laminate those reports......and carry them with me to surgery.......what are my rights to these reports.....I know my Disability Lawyer has copies....as well as the hospital where I failed intubation.......am I entitled to copies? Would I be better off asking my lawyer for the copies......or will the hospital supply them?

I'm trying not to be paranoid......I've just learned, the hard way, that doctors do not always listen to you........
