Looks like you figured it out! This is not an issue with the QME, but with your PCP. I endorse the steps you are taking as the way to go. However, if it were me, being as I have not encountered anything this difficult in records keeping, I might investigate how to approach Doc with your issue and what your recourse is if Doc is not very receptive. I say this because I am one of those "be prepared for all contingencies" kind of gal. Doctors are persuasive and can be intimidating, even with the strongest of us, and I want to see you have the result you seek.

My guess is that since Doc never followed through with recommendations of treatment for those conditions, it would be simpler just to correct the record than to explain the lack of follow-up. Just a thought . . . .
