Yes, please get it checked out and yes they can get worse/more painful.

My dd's dystonia was orginally painless, but as it progressed over a period of several years to become quite painful at times. She also has some tics/tremors/spasms that started out painless, just annoying, and they have become painful if they continue for too long.
