All patients getting methotrexate should be on folic acid.

Studies show using 1mg or more daily of oral B12 is equal to injections. I would suggest the active methyl form called methylcobalamin. It is not commonly found in stores, but is common online, for example at places like iherb.com

Use oral form on an empty stomach.

Cyanocobalamin is synthetic and not natural to the body. Some people have problems with the cyanide portion and cannot convert it. So we have a good source now of the activated form
methylcobalamin.
