I've been where you're at and felt my heart would pound out of my chest when revealing something uncomfortable, and for me feelings are always uncomfortable.
I will share this as well, I was always so proud of myself afterward and each time I learned that it was ok and the earth didn't swallow me whole, I replace the negative thoughts and feelings with a newly learned, more appropriate to the situation response. I had to do that repetively, over years, to replace all the negative. Hon, it was so worth it.
I hope the same for you. I know it's hard. I know it's harder than hard, but if you can make it through it can be so cool. Might I suggest that first you tell your therapist how much anxiety this is causing you and what your body is doing? It can way help to calm.
