that is a long time, but things haven't changed in many regards

wear something you feel good in... dress adequately - but comfortably.

bring a notepad, pen, and resume. i sometimes write prepnotes for myself on the pad too.

focus on your strengths, beforehand and during the interview. during the interview put forth those which you think are most relevant to the job. i used to put these thoughts in my prepnotes.
- be honest about your experiences - but careful not to undermine these. think of them as treasures.

interview them, as well as letting them interview you. This shows you're serious and lets you walk away with more info about the job also. Questions I have asked in the past include: more details on the position (if description murky),
compensation package esp. benefits (mental, dental, eye bla bla bla) coverage, bonuses), about professional growth potential (inside promotion), educational subsidies. i put my questions in prepnotes too. definitely ask for details on health coverage.
