For Each Dx that you have, provide doctor's name, address and phone #. Include all diagnoses and match with all doctors treating the condition. Also, for each Dx and corresponding doctor, be ready to name all medications or treatments. Also, have your work history for the past 10 [I think] years. I am treated for depression, and I was lucky that the local Mental Health and Mental Retardation Center which treats me, filled out my computer form, but I came prepared with the above information. Also, where it asks for additional information you wish to provide, briefly say what areas of functioning, such as work, each Dx impairs. There is more, but if you come prepared with the above, you should be able to provide the other info pretty much off the top of your head. I think you also need your SS card and birth certificate, but you might call your local SS office and ask about this, as I can't actually remember. Best of Luck, and Realize that it's not as complicated as it seems, especially if, like you, one has an "interview" which sounds like they are going to help you with the forms. When the second questionnaire came in - this one mainly about work history, I took it to SS office and they helped me fill it out, as it was a paper form, and I have essential tremor and can barely write.
