This is the Social Security Administration definition of disability. It is doubtful that Vitamin B deficiency will meet that standard. How does your MS affect your ability to perform work related functions? Are you wheel chair bound, do you require crutches & braces, etc.? With physician statements as to the medical need for the assistive devices your case to be adjudicated and allowed overnight.
Don’t be surprised, however, if your application is denied. Your statements strongly suggest you do not have, or even believe you have, a medical condition that meets the SSA disability definition as you indicate you quit work for reasons entirely unrelated to any disabling condition.
If you truly think you have a medical condition that meets the SSA definition of disability you should list every doctor who has treated you for the medically disabling condition(s). You do not need to wait until you have all the paperwork because you will be given plenty of time to modify your application.
I listed all my dx'd conditions, with doctors' addresses and phone #'s, and treatments matched to each. Where there was an opportunity for extra rem arks, I stated how each condition impaired my functioning, e.g. spondalolesthesis impairs my ability to vaccum, rake, sweep, mop and sit for any length of time; depression impairs my ability to initiate and maintain activity of any kind; essential tremors impair ability to write and type; etc. I was approved first time, within 4 months. I take no credit. It was thanks be to God and to the MHMR agency who, due to His influence, helped me fill out the computerized form. You are correct in saying to list ALL conditions. billie
