Dear Waves,
If a hospital is not an option, then my suggestion is something else that involves more intensity than you get now from the pdoc.
I don't know what the something else is.
A group maybe. Maybe not even a group related to mental health support.
I know that you have thought this out.
So I feel silly writing it.
