You may get bleeding under the nail if you have a spur forming on that toe. These spurs form from pressure from too-tight shoes.

You can go to a good podiatrist and they will xray your feet and see if you have spurs under the nails.

Another cause is fungus infection. But spurs by far are the major culprit. Sometimes the podiatrists may recommend removing the nail forever. I passed on this, myself. I have since had one more bleed there under my big toe.
