Hi, I'm so sorry you have a Dr. who obviously doesn't understand RSD and the pain involved. I personally would not be a repeat patient of his. I don't understand insurance policy, but it seems you need a knowledgeable primary for referral or go straight to pain management. I've called my insurance once on a issue that would save them money, they were more than happy to pay for massage therapy rather than a recommended surgery. I had RSD in my shoulder but didn't know it at the time.
