Following up on Septmystic's comments, I turn to one of my my favorite website, that of the RSDSA (Refelx Sympathetic Dystrophy Syndrome Association) and came up with what I hope are three good pages for you, each with numerous sub-pages.
The first is its "Treatment Guidelines" at
http://www.rsds.org/3/clinical_guidelines/index.html There's a lot of material there, once you've opened and printed each one of the pdf files in there. This is basically a small book in it's own right. While perhaps not directly on point, per your request, it's something you should have.
The next page is headed "Resources" and includes a number of articles dealing with workers' comp. and disability claims. While much if not all of this is written around U.S. law, I suspect that 85 to 90% of the stuff regarding claim stratagies will flow across the boarder with complete transparency. And that's here:
